As a new patient, you will be asked to complete a brief registration form. You can download and complete our registration form in Microsoft Word or print a copy and bring with you.

Before you come in, you can use our Web site to familiarize yourself with your Procedure, so that you will know what to expect. You can also use our site to contact the Nurse On Call, or request Prescription Refills. You can even make Credit Card payments securely online.
